Physical Therapist

Summary:
The Physical Therapist (PT) contracted or employed through the Organization is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.

Responsibilities:

  • Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
  • Assesses for muscle strength, mobility, gait, ROM—potential for rehab.
  • Directing physical therapy treatment.

Qualifications:
  • Baccalaureate (BS) degree in physical therapy approved by an accredited organization
  • Licensed to practice as physical therapist within the state, if applicable.
  • One (1) year or more experience as a physical therapist
  • Good verbal, written communication and organization skills.
  • Possesses current CPR certification.

Job Summary:

Job Duties/
Responsibilities:

  1. Evaluates and assesses the patient based on subjective and objective information obtained through record review, formal and informal test administration with appropriate analysis and interpretation of results to develop a plan of care.
  1. Demonstrates professional judgment in choosing evaluation tools.
  2. Demonstrates skill and accuracy in performing patient evaluations and administering standardized tests when appropriate.
  3. Interprets test results appropriately.
  4. Uses clinical judgment in evaluating the patient’s response to treatment, making adjustments regarding patient’s needs.
  5. Consistently observes level of fatigue, pain, motivation, vital signs, frustration, etc., and assesses how long a patient can continue to tolerate activity.

Physical
Requirements:
Ability to lift/carry 50 pounds or more.  Frequent and prolonged bending, stooping, crouching, walking, pushing, pulling, and standing.  Must use oral communications to perform work.  Must use written and/or verbal sources of information.  Must be able to operate a motor vehicle.
Environmental Factors:
Travel is required to make home visits.  Occasionally subjected to irregular hours.

REPORT TO:           Therapy Department Manager 
RESPONSIBILITIES:
1.Provide assessment and following evaluations and administer diagnostic and prognostic tests. Included in the home assessment would be the determination of any methods of devices necessary to improve safety and increase functional independence. Provide discharge planning. Evaluations will be completed on admission, at least every 60 days and upon discharge.
2.Restorative therapy is expected when reasonable potential exists that the therapy will result in material improvement in the individual’s level of functioning in a reasonable period of time. Carrying out a program to maintain function would not be required, but designing a maintenance program, teaching the program to the patient/family and making periodic evaluations of its effectiveness is expected.
3.Select and teach task-oriented therapeutic activities designed to restore sensory integrative function.
4.Teach compensatory techniques such as energy conservation, work simplification techniques to improve the level of independence in activities of daily living, if the service is reasonable and necessary for treating the patient’s condition.
5.Design, fabricate, and fit orthotic and self-help devices if they are related to the patient’s need for occupational therapy services.
6.Complete and turn in clinical and patient Progress Notes, company timesheets and documentation of goal setting in a timely manner.
7.Participates in in-service programs.
8.Assist Therapy Department Manager as needed.
