Helping Hearts Residential Facilities operates behavioral health residential treatment programs in the Valley. We are looking for a compassionate registered nurse to improve coordination of care for our 62 clients. Job duties include:
· Oversight of medication;
· Periodic checks for clients receiving personal care services, including skin assessment, and other health indicators;
· Direct personal care services and ensure compliance to personal care treatment plans;
· Provide coordination of care case management for approximately 40 clients. Maintain Medication Activity Reports
· Assist with coordination between medical personnel, pharmacy and Clinical Liaisons
· Point of contact for medical questions/concerns regarding members
· Complete a yearly nursing assessment of members
· Initial assessment and monthly review of member’s PCS needs
· Assessment and treatment of medical conditions, as needed, for members
· Yearly TB test for staff and members
· Train new hire PCS staff on PCS skills and paperwork
· Ongoing training and review of proper medical assessment, treatment skills and paperwork with PCS staff

Contact all students who have withdrawn from Academy of Art University during a given time period, determined by the VP of Student Operations. Assess the former student’s official standing with the University to best determine if re-entry to the school is appropriate.
Schedule office and/or telephone appointments with former students interested in re-entry to review their status and requirements Assemble and manage information regarding each potential re-entry student Serve as a coordinator with other University departments to complete the file and ensure that the re-entry student is prepared to begin classes Participate in meetings to review the re-enrollment process, track progress and solve problems in academic schedule Advise re-entering students on their progress while in the process of returning to school. Provide updates to other departments on the status of re-entry students. Develop registration and orientation activities for re-entry students Identify and communicate any pertinent information as a result of readmissions activity that may be important to the school's retention effort Partner with advising team to engage students who have expressed an interest in withdrawal.
Work with students, or direct them to other Academy associates as appropriate, to address and resolve their situations in an effort to have them remain in school.
