Role Purpose: Under general supervision, provides professional nursing care to patients within the home setting which includes assessing, planning, implementing, coordinating, delegating and monitoring tasks assigned to other members of the care team in accordance with professional nursing standards. Collaborates with physicians and other health care professionals regarding patient care. Identifies need to change plan(s) of care and facilitates the process using Care Pathways, when appropriate.
Obtains authorizations for visits as appropriate, ensures every visit is reimbursed. Contacts payor case managers as appropriate to discuss plan of care and facilitate authorization for plan of care. Performs ongoing chart review on case load with attention to: a) Accurateness of 485, recertifications, verbal orders for changes in Plan of Care. b) Assuring every visit performed is within assigned frequency. c) Assurings that overall documentation supports coverage of service as defined by regulation or payor. Reports to supervisor when needed.
Completes OASIS documentation timely and accurately with prompt response to supervisors request for corrections.
Demonstrates skill and accuracy with use of Pathways system.
Completes annual competency requirements satisfactorily.
Performs case management & collaborates with other health care providers & delivers results for positive clinical & financial outcomes. ## Qualifications

Job Summary The National Renewable Energy Laboratory (NREL), located at the foothills of the Rocky Mountains in Golden, Colorado is the nation’s primary laboratory for research and development of renewable energy and energy efficiency technologies. A postdoctoral researcher position is available immediately in NREL’s Chemistry & Nanoscience Center to conduct research in the development of conductive carbon nanotube networks, and the application of these networks in electronic devices and energy harvesting technologies.
This candidate will work within a small research to focus on the development of conductive carbon nanotube networks, including strategies focused on optimal doping of such networks, characterization of the electrical transport properties within the networks, and the incorporation of the optimized material systems in novel energy harvesting devices/technologies. Primary duties involve (1) preparing and characterizing tailored semiconducting single-walled carbon nanotube thin film samples, (2) measuring and optimizing their electrical transport properties using transistor-based architectures, and (3) developing prototype energy harvesting devices and characterizing their performance. The candidate is also expected to maintain a strong publication record.
Demonstrated skills and experience in working with solution-processable semiconductors and/or carbon nanotubes, as well as electrical device fabrication and transport measurements. A PhD in chemistry, physics, materials science, or related fields. Experience with sample preparation from liquid and vapor, and characterization techniques such as AFM, scanning Kelvin-probe microscopy, quantitative absorption, Raman, and electrical transport measurements. .
