Nurse Practitioner - Health Risk Assessments - Per Diem
Responsibilities of the Nurse Practitioner- Health Risk Assessments
- Conduct comprehensive in-home health risk assessment to identify all active and chronic disease conditions as well as determine all physical, mental and social needs present at the time of the visit.
- Obtain vital signs, measure BMI, review pharmacological therapy and conduct a physical examination.
- Educate and counsel patient and family on any conditions identified during assessment and screening procedures.
- Make recommendations and communicate findings to Primary Care Physicians through documentation of outcomes on provided tablet.
- Master's degree in Nursing from an accredited school of Nursing.
- Board Certification with current unrestricted Nurse Practitioner (NP) license.
- Experience dealing with a complex set of patients with a relatively high level of acuity.
Physician Practice Manager
Position Summary: The Assistant Practice Manager is responsible to provide on-site practice management services. Responsibilities involve all functions associated with the management of a physician practice including but not limited to: operations, staff scheduling, human resource issues, computer issues/updates, insurance contracting and credentialing, preparation and analysis of key performance indicators, billing and collections review, regulatory compliance, purchasing and inventory, accounts payable and payroll approval, and serve as physician liaison with Hospital.

Seed Consultants, Inc., (SCI) is currently seeking a District Sales Manager for the Upper Sandusky/Marion areas.
The District Sales Manager's responsibilities will include: direct sales to area farmers, recruitment of new accounts, servicing and maintaining current accounts, supporting and servicing SCI's established dealer network, and increasing the territory's overall market share.
