Under the supervision and direction of a registered nurse or an LPN charge nurse, prepares, administers and records select oral, nasal, rectal, inhalant and ophthalmic scheduled medications to residents.
Special Features/Requirements (license, certificates, etc.):
Successful completion of the State of Wisconsin Approved Medication Administration Assistant Course. Must be a certified Nursing Assistant. Has the ability to understand and effectively carry out verbal and/or written instructions. Ability to administer medications to elderly, confused and emotionally upset residents in a safe and competent manner and ability to accurately document this activity. Ability to recognize change in resident condition and ability to communicate and change to charge nurse. Has the ability to communicate effectively with residents and staff. Follow-adheres to organization, federal/state regulations and guidelines, including Resident/Client Rights; maintains neat, well-groomed appearance, adhering to dress code of the organization/program.
Administer all scheduled oral, inhalant, rectal and topical medications, for routine administration according to established facility medication administration polices and procedures
Administer prn medications with supervising nurse's approval
Administer topical treatment creams after they have been shown how to apply
Apply dressings/skin patches such Duoderm and Tegaderm as directed by supervising nurse
Chart administration of prn medication approved by the supervising nurse. Supervising nurse (RN or LPN) must do the follow-up to assess effectiveness
Other duties such as changing 02 tubing, cleaning concentrator filters, etc.
Complies with the Resident Bill of Rights.
Reports to charge nurse when coming on duty and going off duty
Obtains assignment and specific directions from the unit nurse
Reports to the unit nurse any physical and emotional change in resident status immediately
Takes and records blood pressure and pulse as indicated in conjunction with prescribed medication and reports readings obtained
Participates and/or attends as required, all in-service provided for employees as appropriate to the job classification.
Works effectively and cooperatively as a team member with nursing staff
Has access to confidential information/records and maintains confidentiality
Projects a positive image of the organization in dealing with residents/clients, co-workers, and the public
Accuchecks after being checked off skill-wise from licensed nurse
Physical Requirements (based on essential functions):
Frequently pushes/pulls medication carts weighing up to 300 pounds a distance of 400-600 feet; frequently twists, stoops, bends, squats; occasionally kneels, climbs stairs, occasionally reaches above shoulder height; frequently reaches at or below shoulder height; occasionally lifts and carries charts/books weighing under 10 pounds. 100% of workday is spent indoors, 45% of workday is spent standing, 50% walking and 5% sitting.
May be assigned Resident Care Technician duties as needed. Other duties as assigned.
Certified Medication Technician
Our Certified Medication Technician is responsible for all aspects of medication administration within the scope of his/her certification and under the supervision and direction of a Registered Nurse.
High School graduate or equivalent
Graduate of an accredited Certified Medication Technician Course
- One (1) year related experience in a medical environment preferred
Must be certified as a Medication Technician in the state within which services are provided
Must have current CPR Certification
Preparing and administering prescribed medications to patients under the supervision of a licensed RN, and in compliance with all Facility policies and procedures and Pharmacy Board rules and regulations governing the administration and documentation of medication.
Adhering to all Facility policies and procedures and Pharmacy Board rules and regulation regarding the administration and documentation of narcotic medications.
Performing medication administration responsibilities while adhering to all safety principles.
Documenting administration of medication on the Medication Administration Record (MAR).
Documenting medications not administered on the MAR, and the reason for not being administered.

Work under the supervision of a licensed vocational nurse or a registered professional nurse to perform the following duties:
After authorization by the facility's licensed nurse or the resident's treating physician, administers PRN medications.
Organizes assigned medication duties to pour, pass, and document all routine prescribed medications and to give PRN medication only upon the instruction of the licensed nurse.
Must document all PRN's according to State Standards in all areas of the chart.
Observe and report to the facility's charge licensed nurse reactions and side effects of medication commonly administered to nursing facility's residents.
Take and record vital signs prior to administration of medications which could affect or change the vital signs.
Administer and document regularly prescribed medications which the medication aide is permitted to administer only after personally preparing (setting up) of those medications to be administered. Document the medications given in the resident's clinical record.
Administer oxygen per nasal channel or a non-sealing face mask only in an emergency. Immediately after the emergency, the medication aide shall verbally notify the licensed nurse on duty or on call and appropriately document the activity and notification.
At discretion of charge nurse, count narcotics, give all routine prescribed narcotics, and document same on individual sheet and medication sheet.
Must have successfully completed state approved school of medication administration.
Must carry current acknowledgment card or certification issued by state agency.
Must be willing to abide by restrictions placed on his/her job by state agency.
Must be a certified nursing assistant.
Certified Medication Technician
Greenfield of Berryville is currently seeking full-time/part-time certified Medication Technician's to help support the medication program. The Med Tech is responsible for the administration of medications in a safe and organized manner and assisting with resident care. The responsibilities include, but are not limited to: administration of medications, documentation of medication administration, and resident care. The Med Tech does answer to the Health Care and Resident Care Coordinator and will communicate with the pharmacy on a regular basis.
Current medication technician certification (assisted living) in the state of VA in good standing
