Telephonic Chronic Care Management (Health Coach) Coordinator III
Our Telephonic Chronic Care Management (Health Coach) Coordinator III (CNA) position is located in Clearwater, FL. The Chronic Disease Management Program provides Health Coaching on Chronic Diseases (Physical Health/Behavioral Health) supported by the MCG CCGs. The Coordinator/Coach collects patient-centric health information to identify gaps in care and clinical risks. The CDM program supports and educates the patient through the continuum of care to promote the highest level of quality care. Patient education is provided as modules with a frequency agreed upon with the client but are at a minimum monthly with updates provided to the patient-centric Care Plan by the Coordinator.
Essential Duties and Responsibilities:
Educate the patient in the role of the care coordinator with collaboration of primary care physician.
Provide an initial risk identification function gathering all the clinical elements necessary to determine Patient-Centric Care Plan.
Monitor recidivism in ER and inpatient utilization.
Evaluation transition care needs for documented recidivism and implement interventions to address failures of transitional care.
Work with patient to plan and monitor their care as determined by the PCP and/specialist.
Promote compliance with transitional care, PCP/Specialist office visits.
Promote compliance with medication (medication adherence).
Act as a patient advocate, liaison and information resource.
Provide modular/formal chronic disease education for all chronic/complex conditions specific to the patient's clinical diagnoses
Gather and report outcomes to Case Manager's Care Plan goals and interventions with appropriate frequency
Facilitate patient access to appropriate medical and specialty providers as defined by the PCP.
Cultivate and support primary care and specialty provider who determine the care needs of the patient.
Provide culturally appropriate education.

The Software QA Tester will be required to join the centralized IT QA/QC service to standardize quality best practices, raise the overall quality of delivered solutions, and shift testing workload off developers, project managers, and business analysts.

Serve as a subject matter expert and advocate for IT QA/QC to promote a culture of good quality practices.
Aid in the recommendation, evaluation, selection, and implementation of appropriate tools to enable the strategy for the maturation of IT QA/QC.
Excellent time management, planning, and multi-tasking skills to support more than one testing campaign at the same time.
Excellent communication skills (verbal, written, phone, video, etc.).
High School diploma or GED and a minimum of 3 years' manual software testing experience (writing, executing and reporting functional test cases) in a banking environment.
2+ Years' specific experience with testing of SalesForce deployments.
1-2 Years' experience testing nCino with SalesForce preferred.
Experience working in an environment that has evolved from manual software testing to automated software testing preferred.
Experience with test automation tools, methods, and protocols preferred.
Experience with functional testing in an Agile development methodology preferred.
CSTP or CTFL certification preferred.
