INTAKE COORDINATOR, ACSW/MFTI

Reports To: Team Leader

Category: Administrative/Direct Service
Responsible for all inquiries from the community, DMH and/or family members; engage referrals, conduct clinical and needs assessments triage to MHALA and community resources. This is both a direct service and an administrative position.
Essential Duties and Responsibilities:

Provides clinical consultation and support as needed and requested by the staff. Provides and supervises intakes and initial assessments
Notifies Team Leader of urgent and non-urgent referrals
Coordinates movement of people from one doorkeeper to another and consults on and mediates decision making
Manages phone inquiries dispersing when appropriate and/or by responding to the inquiry
Provides support to other staff involved in initial contact and directly (and indirectly) provides services to prospective members while they are in the outreach /transition phase
Maintains all records and documentation needed for program compliance and accountability in accordance with MHA standards and funding source requirements
Facilitates MHA relationship with referral sources / community
Oversees MHALA OC referrals, tracks outcomes and looks for a need to expand the menu of services if called for
Explain governmental benefits, requirements and procedures, including eligibility requirements to
members and fellow staff
Completes all paperwork and maintains all records and documentation required for program compliance and accountability in accordance with MHA standards and funding source requirements
Other position requirements:
Masters Degree in Social Work (MSW) with ACSW or Masters Degree in Psychology with Marriage and Family Therapy Emphasis (MFT) with MFTI registration number required.
Participate in program designed for Licensed Waivered staff to attain supervised hours and be willing to pursue Licensed Clinical Social Worker status or Licensed Marriage and Family Therapy status. All billing and supporting documentation shall be entered into MHALAs EHR (electronic health record system) within 24 hours from the date of service delivery.
All required data collection documents (as applicable to your program: MORS, Determinants, KEC, IHOMS, HMIS, etc.) are submitted on or before due date.
Ability to provide a welcoming environment and skill in engaging new members.
Must function well in a team-oriented approach to member services.
Must be familiar with federal, state and local housing subsidy programs and the sources available for them.
Must be familiar with the practices and procedures for preparation of applications for subsidized housing and disabled/low income housing loans.
Knowledge of community resources.
Knowledge of, and dedication to, the Psychosocial Rehabilitation Model of mental health treatment, community integration, and hope in recovery.
Ability to work collaboratively and effectively with public and private housing providers, mental health consumers and their family members or client advocates, vendors and other staff.
Excellent writing skills, including the ability to summarize comprehensive diagnostic and biopsychosocial information in narrative form. Ability to complete biopsychosocial Assessments.

Knowledgeable in computer applications such as Microsoft Word, Excel and Outlook
Driving with current class C drivers license, a vehicle for transportation, proof of valid automobile insurance, the ability to be insured by MHA liability insurance are conditions of employment.
