LPN, provides safe, therapeutic and efficient care and services to patients. Functions as a member of the health care team at an intermediate nursing level to assist in the planning and implementation of patient care based upon sound nursing judgment, clinical preparation, knowledge, skills and experience.

2 In collaboration with Physician/APC, reinforces health education, counseling, and advice to patients and families on topics such as healthy life style changes. Effectively assesses patient educational needs, prioritizes patients learning needs, identifies barriers to learning, and uses various educational tools to encourage the non-adherent patient.
3 Tracks, reviews, and monitors the follow-up of diagnostic test results to ensure the physician/APC have verified results and patient has been notified. Provides related education, counseling, ongoing review, and monitoring regarding diagnostic tests (i.e., INR results in Anticoagulation Clinic) under the collaboration of the Physician/APC. Assists in the management of patient care between visits.
4 Assesses patients/callers health status based upon presenting symptoms and follows defined protocols. Determines the urgency for care and directs patient/caller to the most appropriate level of care. Should access not be readily available at the practice, will direct patient/caller to an acceptable alternative venue of care. Seeks Physician/APC advice appropriately.
Performs the responsibilities of the Physician Office Nurse as outlined below:
5 Obtains and documents reason for visit and patient vital signs to include both pain scale and pulse oximetry as appropriate.
6 Assists Physician/APC with examinations, procedures and other processes related to direct patient care.
7 Collects specimens as directed by Physician/APC. Uses appropriate equipment and protocols for collecting, handling, testing, labeling, and preparing for transportation to lab including proper identification, storage procedures and techniques.
8 Ensures all patient care areas are adequately stocked with unexpired supplies and medications.
9 Prepares patients for scheduled procedures by assuring patient is undressed and appropriately gowned.
10 Assesses sterility of instruments, ensures they are intact, and supplies are set-up.
11 Sterilizes instruments, equipment, and supplies as required, when applicable. Handles and stores instruments, equipment and supplies appropriately.
12 Completes necessary paperwork, prior authorizations and/or scheduling of appointments for patients who are ordered diagnostic and therapeutic services.
13 Maintains patient records to ensure accurate documentation of the patients medical treatment.
14 Adheres to Health Insurance Portability and Accountability Act (HIPAA) of 1996.
15 Prepares and administers vaccines, medications, and controlled substances/narcotics as prescribed by the Physician/APC and as certified competencies permit. Utilizes the Six Rights of Medication Administration. Asks prescribed screening questions, identifies risks and contraindications, questions and observes for appropriate responses or adverse reactions.
16 Documents and processes prescription requests and refills per Physician/APC order and or policy.
17 Uses Universal Precautions with every patient and visitor encounter.
Minimum Education: Certificate of completion (degree or diploma) from an accredited LPN program
Minimum Experience: 1 – 2 years of previous clinical nursing experience
Required Certification/Registration: Licensed Practical Nurse; BCLS certification.

Job Responsibilities and Accountabilities
Comply and follow all Federal, State, Local, Industry and Acme Inc. policies, ordinances, standards and procedures such as use of Personal Protective Equipment, Lock out/Tag out, Hot Work Permits, FM Global Procedures and PSM/RMP.
Must abide and enforce all Acme safety standards and requirements including personal protective equipment need to perform job duties. Conduct monthly safety meeting with the maintenance departments.
Must comply and maintain Acme Risk Management Evaluation program (RCME) and support Sanitation programs to accomplish and audit score of 900 or better.
Delegate and track and track daily repair task to Supervision and or maintenance staff and verification of work complete.
Schedule and review all preventative maintenance and repairs for facility, life safety, ground, refrigeration and material handling equipment.
Utilize computerize maintenance system. Verifies work orders to ensure that they are properly prepared and completed correctly.
Experience with building blueprints, electrical, plumbing, heating, and ventilation systems.
Experience with material handling equipment.
Schedule Contractors; maintain schedules, project plans and timelines.
Assist training for both Supervisors and maintenance associates and ensures that preventative maintenance procedures are in effect and are being performed on a timely basis.
Facilitates and delegates inspections as necessary.
Verify correct inventory procedures were followed. Order parts, supplies or tools to accomplish all jobs.
Motivated and self-directed to complete workload assigned in a timely manner.
Relevant Experience, Education, Certification, Knowledge, Skills and Abilities
High school degree required. College or trade school degree desirable, work experience in areas of refrigeration and electrical maintenance required.
Working knowledge of the computerized maintenance program to document and schedule all maintenance tasks and inventories.
Possess good leadership skills and the ability to supervise the work of others with good time management skills. Must be able to motivate and work with and through others to achieve desired results.
Possess good communication skills, both verbal and written. Deal effectively with a wide variety of people both in person and over the telephone.
Basic computer skills required and personal computer experience, with word processing and spreadsheet applications preferred.
Requires the analytical ability to handle administrative duties and mental alertness to ensure accurate, safe, and thorough completion of work activities. Possess ability to concentrate and deal with frequent interruptions.
