DSS LPN position-must be able to work some weekends and some holidays as per DSS Guidelines. SUMMARY OF THE JOB Provides nursing care under the supervision of a Staff RN and delivers care consistent with identified plan and standards of care.
Reports observations and outcomes of care to the case managing Staff RN. Performs personal care needs as the plan of care directs. Role involves both skilled and unskilled patient visits ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Job Duties#include the following, other duties may be assigned:
Performs treatments and procedures appropriate to the scope of practice and accurately documents care in patient record in a timely manner. Utilizes interdisciplinary care plan as developed by the Staff RN and other members of the interdisciplinary team.#Provides feedback to the Staff RN for consideration in developing care plans to improve patient care outcomes. Is responsible to participate with weekly case conferences as part of the interdisciplinary team.
Informs designated Staff RN of current developments, problems, significant conditions and symptoms utilizing proper communication system. Assists with resolution of problems as directed by the Staff RN. Follows written and verbal instructions from a Staff RN.
Is responsible for coordinating patient care with the Staff RN to promote continuity and for communication with other departments as appropriate. Administers medications according to approved procedures and documents accurately in patient record, excluding medications in the following categories which must be given by a Registered Nurse: any intravenous push medications, intradermal medications, non oral cancer chemotherapeutic agents, and investigational drugs. Demonstrates awareness of medications# actions, adverse reactions, and side effects.
Documents nursing care and other significant information in a timely fashion. Maintains a clinical record that meets the requirements of policy and procedure and accepted standard of practice for each patient receiving care. Works with team members in scheduling a daily patient case load that is reasonable, responsible, and promotes continuity of care.
Reports problem areas to manager. Improvises and manipulates the patient#s environment so that skilled nursing care can be adequately provided. Conforms to established Agency expectations of productivity.
Assists in the orientation of new personnel and/or student nurses. Refers to and follows standards of policy and procedure manuals, including specific departmental manuals. Attends meetings and/or workshops as requested.
Demonstrates awareness and support of the Agency#s outcome objectives. Accepts responsibility under adverse working conditions, within the limits of safety. Respects personal and medical patient information in strictest confidence.
Participates in the weekend and holiday schedules as assigned. Driving is required for visiting staff. Performs all other duties as may be required.
