Medical Social Worker - Social Worker - PRN
What You Will Do:
As a Medical Social Worker (MSW) for Amedisys, you will have the opportunity to assist and counsel patients and their family one on one with health related financial, social and emotional concerns. Your work will include assisting the physician and other members of the patient care team to understand the significance of social and emotional factors related to the patient's care. You will contribute to the development of the plan of care with the physician, patient, family and staff to best manage personal, financial and environmental difficulties that may interfere with the health and wellness continuum.
Counsel patients and families concerning the facilitation of the medical Plan of Care.
Recognize, identify and utilize community and family resources to assist the patient in long-term plans for life in the community or to learn to live within the limits of the patient's disability.
Participate in discharge planning.
Coordinate the total Plan of Care and maintain continuity of patient care by liaising with appropriate staff.
Attend and actively participate in patient care conferences to ensure good coordination of care. Develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies.
May supervise Social Worker Assistants as a licensed social worker
Required:
Master's degree from a school of social work accredited by the Council on Social Work Education
A current and unencumbered license or certification to practice as a social worker in the state assigned to work with some limited state specific exceptions*
One (1 ) year or more experience as a social worker in a healthcare setting
Current CPR certification
Must have reliable transportation, current driver's license, and agency-required liability insurance to perform home visits
Preferred:
- Previous home health or hospice experience

Employment Qualifications
Minimum Qualifications
Either: 1a. Two years of full-time, paid experience employed by the County of Sacramento in the class of Human Services Specialist;
AND
1b. Successful completion of 12 semester or 18 quarter upper division units from an accredited college or university in social work, sociology, psychology, counseling or other field closely related to the intent of the class.
Or: 2. A Bachelor's Degree or higher from an accredited college or university with at least 24 semester or 36 quarter upper division units in social work, sociology, psychology, counseling or other field closely related to the intent of the class.
Note: Related fields above may include behavioral science, child development, community health education, cultural anthropology, ethnic studies, family studies, gerontology, human development, mental health, and public health.
Note: Part-time experience is converted to full-time on the basis of 173.6 hours = 21.7 days = 1 work month.
Note: The word "experience" referenced in the minimum qualifications means full-time paid experience unless the job announcement states that volunteer experience is acceptable. Part-time paid experience may be accumulated and pro-rated to meet the total experience requirements.

General Qualifications
Criminal History and Background Checks:
The County may access criminal history information on candidates who have accepted a conditional offer of appointment for this class consistent with the provisions of Board of Supervisors Resolution No. 82-602, Personnel Policies and Procedures B-5, and applicable federal and state laws. The County shall not consider for employment any candidate who has been convicted of a felony or misdemeanor that relates to or impacts the candidate's ability to perform the job duties of this class unless it is determined that mitigating circumstances exist. For purposes of accessing criminal history information, the candidate will be fingerprinted. A subsequent arrest notification may be obtained.
The County may also conduct a background check on the candidate prior to appointment to a position within this class. The background check may include personal and professional reference checks, credit history checks where authorized by Labor Code section 1024.5, Social Security Number verification, professional license/registration verification, military service information and driving history. Information obtained in the course of this background check will be considered by the appointing authority in the selection process. In obtaining such information, the County will comply with applicable consent and disclosure practices in the Fair Credit Reporting Act and the California Investigative Consumer Reporting Agencies Act.
Driver's License:
A valid California Driver License, Class C or higher, may be required at the time of appointment. Failure to obtain or maintain the appropriate California Driver License may constitute cause for personnel action in accordance with Civil Service Commission Rules or applicable bargaining agreement. Individuals who do not meet this requirement due to disability will be reviewed on a case-by-case basis.
Child and Elder Abuse Reporting:
Persons selected for employment must, as a condition of employment, sign a statement agreeing to comply with Section 11166 of the California Penal Code and Section 15630 of the Welfare and Institutions Code relating to child and elder abuse reporting.
Physical Requirements:
Some positions in this class may require the incumbent to be able to:
Sit for periods up to one hour while driving to home visits.
Climb flights of stairs to reach residence.
Lift infants and toddlers weighing up to 40 pounds.
Carry automobile child safety seats.
Have sufficient vision to assess physical condition of clients and home environment.

In addition, incumbents translate and interpret using standard English and a language other than standard English, and act as a consultant to others regarding the specific multi-cultural group.

Job Summary
This is a notification for an open 32-hour clinical social work position in Community Health Services, Regional Alliance for Healthy Schools. The appointment is at Ypsilanti Community Middle School.
Primary duties include mental health assessment; direct psychotherapy (individual, family and group) with at risk adolescents and their families using evidence based therapy models (CBT, IPT, DBT, TF-CBT) to facilitate an understanding and resolution of identified problems; facilitation of youth advisory council leadership development and activities; provision of referrals and follow-up to community resources as needed; specialty consultation and networking with teachers/school staff; and provision of mental health education to the targeted population.
This position requires experience with providing evidence based therapy models, full licensure, ability for independent decision making, ability to communicate with and work effectively on a multi-disciplinary team, accurate and timely collection and reporting of data; and participation in the education and training of students and staff from social work and other disciplines.
Required Qualifications*
Master's degree in Social Work from a school accredited by the Council of Social Work Education is necessary. Must have current clinical license to practice social work in the State of Michigan. Clinical practice concentration and field practicum or recent work experience in a hospital required.

Responsibilities
The incumbent provides direct supervision to federal inmates of various ages and skill levels assigned to the Textile Factory. All inmates must be trained by the incumbent to cut, assemble and package a finished product which meets rigid Department of Defense and other government and private sector specifications in sufficient quantity and quality.
The details vary in size and the duties cover such diverse areas as supervising cutting processes (e.g., pattern & marker development, spreading, cutting and shade numbering), sewing processes (e.g., setting collars, hemming, side seaming, button hole, manual operations), packing processes and clerical functions.Since the position is located in a correctional environment and the incumbent supervises a workforce comprised of federal criminal offenders, law enforcement responsibilities compound those duties normally related to a manufacturing operation.Along with all other correctional institution employees, incumbent is charged with responsibility for maintaining security of the institution. Minimum Qualifications
Ability to read and write. A minimum of six months of paid experience in general laboring work. Requires a valid State of Ohio driver's license and may require possession of a valid State of Ohio commercial driver's license (CDL class and endorsements as required by division).
Basic knowledge of the methods, tools and equipment used in labor operations.
Ability to use small machines, hand and power tools, and less complex power equipment.
