Hospitalist group in Georgia is seeking a board certified/board eligible Internal Medicine physician to practice Hospitalist medicine as a Nocturnist. The hospitalist will experience a broad pathology of patients and physician activities that includes patient care, teaching, research, and leadership related to hospital care. As expected, hospitalists manage patients throughout the continuum of hospital care, often seeing patients in the ER, admitting them to inpatient units, following them as necessary into the critical care unit, and organizing post-acute care and transfer back to their primary care physician.
- Up to to twenty-two shifts a month.
- Rounding shifts are usually fifteen to sixteen times a month and average six to eight hours each shift.
- On Call Shifts are twelve hours long with approximately five to six shifts per month.
- Malpractice coverage is paid by the practice. The practice will pick up prior acts coverage if needed. Tail coverage is also available.
- The physician will initially be hired as an employee with potential advancement to partner.

The Nurse Practitioner is a licensed independent practitioner who is responsible for managing health problems and coordinating health care for primary care patients in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). Work is performed under the general supervision of the Attending Physician in charge of patient care who will review work routinely. Essential Duties and Responsibilities*
1. Obtains and records client's health history; performs comprehensive physical assessment (including biological, social and psychological needs) as part of a total physical examination.
2. Diagnoses, treats and plans appropriate health care for clients in the area of primary care for patients in private homes, assisted living facilities, personal care homes as appropriate; refers clients to physicians and/or outside agencies as required.
3. Provides and prescribes medications within area of specialty.

5. Provides case management services for select clients.
6. Establishes and maintains client’s electronic medical records and other necessary records.
7. Provides in-service education classes on assigned responsibilities for nursing staff as needed.

9. Perform urgent care as necessary for mobile integrated health systems for local health systems.
10. Provide comprehensive post discharge follow up and implement a 30 day plan of care for transitional care; provide assistance with transitioning patients back to their primary care providers if not a current patient of HCMA (in the form of appointment calls, case management or social needs, DME orders and medical record transfer).

Necessary special requirements: Licensed by the State of Georgia as a Registered Nurse; certified by the State of Georgia as a Nurse Practitioner; Prescriptive privileges; DEA; possession of a valid Georgia Driver's license without restrictions.
Experience with providing primary care in private homes, assisted living facilities and personal care homes preferred.
