Assists with feeding, ambulation and personal care if assigned to do so

Completes a skilled nursing note as services are performed and submits these for incorporation into the clinical record within 24-48 hours unless

We have 19 retail operations in Louisiana, Texas, and Arkansas, with packaged gas production capabilities in Shreveport, LA, Dallas/Fort Worth, TX and Houston, TX.
1. Must possess a current CDL Driver’s License with hazardous material endorsement.
2. Must possess history of safe driving record exhibited by annual MVR Reports.
3. Must pass required annual Physical Exam required by D.O.T.

· Following the directions of the Dispatcher or Supervisor.
· Taking the most direct and advantageous route possible.
6. Ensures the accuracy of deliveries and pick-ups by filling out the delivery tickets properly and double-checking all items before leaving and with customer upon delivery.
