Lay Patient Navigator
The Navigator will be responsible for advancing patient education, improving patient and provider communication, enhancing satisfaction, promoting better utilization of clinical services and streamlining coordination of care. This employee is an essential link between patients, their physicians, and other specialty care providers. This individual will be responsible for the coordination of the administrative, operational, and patient navigation functions of the Breast Center. The goal is to ensure patients effectively and efficiently transition through the continuum of breast cancer care from newly diagnosed through long-term survivorship receiving appropriate services, support, and education. This individual will function as an integral member of the Breast Team, collaborating with patients, survivors, volunteers, providers, and outside health agencies related to breast cancer. Working closely with the Program Director, Program Manager and other educators/navigators, this individual will work to build on the already existent multidisciplinary programs devoted to prevention, diagnosis, care, and treatment of patients with breast cancer.
Specific Duties and Responsibilities:
Scope of Responsibility:
Knows the formal and informal university and departmental goals, standards, policies, and procedures. Will function independently and will serve as a representative of the Breast Center, and as such, must demonstrate a high standard of professional conduct and political acumen.
Patient/Family Support and Navigation:
* Follow patients throughout the care continuum from diagnosis through long-term survivorship to meet ongoing education and navigation needs.
* Work with local cancer support programs and outside organizations and provide seamless referrals and collaborations.
* Oversee and manage support group activities.
* Assess the level of need for navigation for patients and caregivers and provide support where necessary.
* Facilitate consults and support service referrals including social work, nutrition, and other specialty providers at Johns Hopkins and in the community.
* Assist medical providers in ensuring patient-centered care along the continuum.
* Be a point of contact for patients who need follow up, questions answered or to make connections with other health providers.
* Participate in the development of brochures, questionnaires, newsletters, handouts, web content, and other written educational materials.
* Participate in grant writing to increase programmatic funding.
* Assist in preparation of summaries, presentations, and various reports for sponsors.
* Assist with donor interactions and required philanthropy proposals/project summaries.
Patient Education and Outreach:
* Participate in event planning related to support activities of the Breast Center including an annual Survivorship Day.
* Develop, plan, and maintain our educational programs such as webinar support, online nutrition courses, caregiver events, and our educational series.
* This will include the marketing and creation of materials for these events.
* Develop, plan and implement outreach programs for the local community that encourages early detection, risk factors, healthy behaviors, and resources.
* Identify and suggest advance strategies for project improvements.
* Work independently to ensure program quality and adherence to expected processes and project goals and timelines.
* Create and maintain up to date educational materials including informational sheets, website material, newsletters, webinars, and videos.
* Coordinate marketing activities for the Breast Center including local, regional, and national public speaking engagements.
* Willing to work at/between multiple sites (East Baltimore and Green Spring Station) and build multi-site collaborations.
* Participate in and coordinate fundraising initiatives related to supporting activities, particularly as it relates to funding for outreach and education programs.
* Attend regular Breast Center meetings.
* On a regular and continuous basis, exercises administrative judgment and assumes responsibility for decisions, consequences, and results having an impact on people, costs, and/or quality of work being performed under the auspices of the Breast Center.
* Exchanges information using tact and persuasion as appropriate requiring excellent oral and written communication skills.
* Demonstrates ability to effectively interact and develop liaisons with multiple layers of personnel at academic, private sector, and public/governmental agencies.
* Excellent presentation skills necessary.

* Bachelor's Degree in a health or social services related field.
* Three (3) years' experience in oncology, social services, and/or public health.
* Knowledge ina medical setting sufficient to attain the skills required.
* Master's Degree in public health, social work or other related field.

* Familiarity of various local, state, and national breast cancer awareness, education, outreach, and advocacy programs and organizations.
* Proficiency in writing technical and programmatic reports that document program direction or results.
* Ability to interact skillfully and diplomatically with numerous counterparts, working cooperatively with colleagues who represent a wide range of interests and needs.
* Ability to promote consensus building by soliciting thoughts and opinions from other team members through surveys and other means of communication.

Starting Salary Range: $52,495 - $72,210 (commensurate with experience)
