Under the supervision of a registered nurse, provides basic care to clients in their home to maintain optimal physical and emotional comforts.
Assists in providing and/or maintaining optimal physical and emotional comforts to clients and their families.
Provides clients with personal care and activities to include bathing, oral hygiene, skin care, toileting, hydrating, feeding, ambulating, changing of bed linens and repositioning.
Assists clients and their families toward achieving maximum self-reliance within their environment.
Turns client in bed; transfers client between bed, chair, and commode.
Prepares meals, including those requiring special diets when indicated.
May accompany client to the physician's office or clinic, etc..
May perform basic non-sterile treatments and procedures under the supervision of the registered nurse.
May assist clients with exercise and range of motion according to the care plan developed by the registered nurse/therapist.
May remind clients to self- administer medications.
Keeps registered nurse informed of the client's progress and reports all changes in the client's condition. Receives and gives report on assigned client with other shifts as appropriate.
Submits clinical documentation on a timely and accurate basis.
Participates in in-service and orientation programs and other required meetings.
Demonstrates a commitment to maximizing the satisfaction level of all internal and external customers through utilization of quality improvement principles.
Maintains a safe work environment and practices personal safety techniques to prevent injury to self and others.
Performs other related duties as required. EDUCATION & EXPERIENCE:
Requires a high school diploma or GED.
Requires at least one year of recent hospital, nursing home or home health care experience.
Ability to pass the NLN Skills Assessment Test with a score of 75% or better.
Ability to pass Technical Skills Checklist. Completion of a certified nurse aide training program is preferred with one year of recent hospital, nursing home, or home health care experience.
BCLS certification is preferred.
Must possess a current State of Michigan drivers license and a satisfactory driving record.
Must have access to reliable transportation and a telephone.

ENTRY REQUIREMENTS Graduation from a recognized school of nursing and a current Michigan RN licensure. Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health prior to January 1, 2009 are encouraged to obtain a BSN.
Certification in the specialty area of practice is also encouraged. Registered nurses hired by Munson Home Health or transferred from Munson Medical Center to Munson Home Health on or after January 1, 2009, are required to obtain a BSN within ten years of their system hire date. Certification in the specialty area of practice is encouraged.
Home care, hospice, or private duty nursing experience preferred. Demonstrated health assessment, organizational and interpersonal skills at an expert level. Proven self-directed professional with ability to make independent judgments directing patient care.
Proven ability to communicate effectively in orally and in writing. Possesses current Michigan motor vehicle license, ability to drive a car, and an insured vehicle capable of transport to patient homes or other offices in various weather conditions. Meets MHH established requirements for proof of health status.
Physically able to lift 35 pounds. Demonstrates ability to move, position and transfer clients utilizing good body mechanics, lifting techniques and/or assistive devices to avoid manipulating more than 35 pounds. Meets Home Health and Medicare standards as evidenced by criminal background check and fingerprinting.
CPR certification preferred. ORGANIZATION: Reports to the manager or designee. Receives direction from the Service Coordinator.
Provides direction to and supervision of Aides, LPN's and homemakers. Interacts interdependently with other Home Health staff and volunteers, physicians, support staff, other hospital staff and members of the community. Nurses without acute care experience may be hired for specific hourly nursing cases under the supervision of a service coordinator.
