Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
One year of nursing experience to include ventilator, wound care and respiratory care
Experience is required within the last two years as a registered nurse.
BSN and/or Master's degree from an accredited school of nursing recognized by the VA (Commission on Collegiate Nursing Education (CCNE) or The Accreditation Commission for Education in Nursing (ACEN).
National Certification in specialty area.
Preceptor training within the last two years.
Ability to exercise critical thinking, problem solving and collaborative skills in a high stress environment.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

Physical Requirements:
Involves considerable amount of standing, walking, stooping, bending, reaching, lifting, and turning.

The position requires visual and auditory acuity at a level to provide safe and effective nursing care.
The position requires the nurse to work for periods of time with arms above shoulder level or with neck in a fully flexed or extended position and the ability to physically control or defend against emotionally ill clients.

The Community Treatment Team program provides outreach and community based mental health services to individuals with serious and persistent mental health illnesses.
The Professional Staff Nurse is a Registered Nurse is a member of the care delivery team. The Professional Staff Nurse is responsible to set the standards for the level and quality of care. The Professional Staff Nurse has responsibility, authority and accountability for the provision of nursing care. Physical Requirements:
This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This is a professional public health nursing position in the Sexually Transmitted Disease (STD) program. The incumbent is assigned to perform a clearly defined clinical function permitted by state communicable disease guidelines. Maintains an organized system for following up with clients. Assists in the follow-up of communicable and sexually transmitted diseases, abnormal pap tests and refers to appropriate agency as needed. Counsels and supports clients/families that have experienced HIV or any devastating disease.
Acts as liaison between client and community agencies. Registered nurses shall be responsible for the following activities as consistent with the nurse practice act: history taking, physical assessment, care referral, counseling/education, health promotion and risk reduction counseling, and referring patients that require disease intervention counseling to the DIS.
The RN may also perform sample collection for lab testing. All complex and multi-problem cases shall be referred to licensed clinicians for collaborative case management.
Assesses plans, develops, implements and evaluates treatment programs that promote health and well-being; treatment plans for patients with a known long-term condition; and treatment plans for patients at risk of developing a long-term condition as appropriate. Works with other health care professionals to diagnose, monitor, manage and treat long-term conditions, including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs.
Assesses complex patient needs/situations and consults with Physician and/or Advance Registered Nurse Practitioner as needed. Additionally, nurses shall be responsible for tracking of abnormal Pap smear follow-up when done as part of a diagnostic procedure for the STD determination, referrals made to other providers and the supply of pharmaceuticals retained in the clinic settings.
Assists in the planning process and orientation of new staff and provides clinical experience for university nursing students. Represents local STD unit on various committees within the agency and the community.
Coordinates STD clinic activities in the absence of the supervisor. Relieves nurse in other STD clinic when needed.
Conducts peer reviews of medical records. Monitors inventory of pharmaceutical supplies and works with the Asst. CHN Director to ensure supplies and medications have been ordered.

Qualifications
QUALIFICATIONS
Graduation from an NLN accredited school of nursing: BSN preferred.
A current New York State License to practice as a Registered or Licensed Nurse.
A minimum of one year's general nursing experience with current home health, medical surgical or critical care experience.
Possess and maintain good physical stamina and mental health, including satisfactory health clearance as required by NYS regulations and Agency policy.
Current Driver's License, required insurance and car available for work as required.

This is a responsible nursing position in the Madison County Health Department (CHD) that provides a variety of community health nursing services. This position supports clinical nursing coverage, provides oversight of the school health program and supervises school health staff. This position is responsible for the supervision of staff including appraisals, planning and directing their work, approving leave, and having the authority for hiring and taking disciplinary action or effectively recommending such actions. Responsibilities include, but are not limited to:
School Health: Provides direct supervision of Licensed Practical Nurses and Health Support Technicians, plans and assists in mandated health screenings, provides care plans for students with diagnoses requiring care, supervises distribution of prescription and over-the-counter medications, prepares annual School Health Report and bi-annual School Health Plan, meets with School Health staff on a frequent basis to ensure adherence to guidelines, monitors high risk students for compliance and follow-up through medical referral, and promotes the School Health Program throughout the community
Clinic Activity: Responsible for the CHD clinics including General Clinic, Primary Care Clinic, OB Clinic, Family Planning Clinic, Immunizations, Walk-In and Triage, as well as programmatic activities ensuring that each program meets all state requirements.
Renders skilled nursing assessment to uncover health problems through observation, interviews and record analysis. Administers preventive immunizations and treatments under medical direction, following written protocols and procedures of the CHD. Assists Physician and/or Advanced Registered Nurse Practitioner (ARNP) with examinations and treatments.
Obtains laboratory specimens and performs diagnostic tests following written guidelines and procedures. Maintains electronic health records via the Health Management System with proper documentation by recording all services and interactions with patients.
Triage: Provide assistance with patient referrals through other agencies and clinics, assist provider with patient follow-up as indicated ensuring complete and accurate documentation on all patients.
Answers and Triage incoming phone calls promptly redirecting calls as needed. Scan patient records into the Health Management System (EHR System) as needed.
Other: Attend workshops, meetings, seminars and in services for professional growth and development.
Maintain continuation credits and re-licensure. Maintains current CPR card. Maintain leave and attendance records via People First, Employee Activities Record via the Health Management System and submits to appropriate supervisor in a timely manner. Performs other related duties as assigned.

Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
