Principal Responsibilities1. JOB SUMMARY Assist in the comprehensive orthotic and prosthetic patient care under the direction of a physician's prescription. This includes patient assessment, formulation and implementation of a treatment plan, as well as follow-up and practice management. POSITION RESPONSIBILITIES NCOPE accredited residency in Orthotics and Prosthetics, resident will work with credentialed staff evaluating, casting, fitting and adjusting orthotic and prosthetic devices to fulfill residency requirements.
QUALIFICATIONS Must be registered with NCOPE prior to starting the residency program. Must possess a valid class D driver's license.

JOB SUMMARY: Provides personal care assistance to participants at the Adult Day Health Center (ADHC) and provides personal care, homemaker services, personal interaction and restorative care to participants in their homes to maintain a healthy and independent lifestyle in the community. The Personal Care Assistant (PAC), works independently in a safe and courteous manner, under the direction of a care plan developed by Home Care Coordinator and Participant, to provide care needed, allowing the maximum level of independence in the home.
The Personal Care Assistant is required to go to assigned interdisciplinary team and scheduled monthly meetings, and complete all required educational trainings. The PCA reports directly to the Home Care Coordinator. In the absence of the Home Care Coordinator, the PCA reports to the Center Manager.
MAJOR DUTIES AND RESPONSIBILITIES: *1. Provides direct Participant Care a.
Personal care (bathing, dressing, grooming, toileting, skin care, foot care, oral care, nail care, etc.) b. Nutrition/hydration (Preparing, serving and clean up of meals) and assist Participant with feeding as needed) c. Obtain Vital Signs (Temperature, Blood Pressure, Pulse, Respiration) d.
Blood glucose monitoring as needed following successful competency assessment. e. Applying orthotics devices (braces, elastic stocking, etc.) f. Assist with ambulation and transfers of participant and active and passive range of motion. g.
Assist with prompting for medication compliance. h. Care of participant incontinence. *2. Participates with indirect Participant Care responsibilities including but not limited to: a.
Light Housekeeping. b. Assist with Laundry. c. Shopping (a few items). d.
Use supplies accurately and economically. e. Delivery of medication and supplies as needed. f. May require on-call coverage for after hours care (5 PM to 8 AM and week-ends) for emergencies in home. g.
In ADHC assist with recreational activities. *3. Remains attentive to Participant and Personal Care Assistant safety responsibilities. a. Appropriately utilizes good body mechanics and use of gait belt, back brace for transfers and ambulation of participants. b.
Demonstrates understanding and utilizes appropriate devices for transfers and ambulation. c. Immediately reports change of condition of participant to Home Care Coordinator, Clinic RN, or Center Manager. d. Immediately reports unsafe condition in the home to Home Care Coordinator. e.
Immediately reports to Supervisor if unable to get in to do participant visit. Provides notice per policy stating you (PCA) were there. f. Demonstrates understanding and utilizes Universal Precautions. g.
Demonstrates understanding and utilizes Emergency Procedures. h. Completes annual competency trainings and skill assessment(s) as required. i. Escorts participant to outside appointments and returns information from appointment to Clinic Nurse. j.
Assist transportation with loading and unloading of buses at the ADHC. k. Collects data for mandatory requirements and various quality improvement projects and is actively involved in action plans and implementation to improve quality for the LIFE Geisinger program. *4. Completes accurate and thorough documentation for services provided according to program policy. *5.
Maintains effective communication skills with the older adult and with co-workers. a. Explains actions to participant before starting task. b. Understands/demonstrates that all care is done with full regard to participants rights. c.
Provides orientation and redirection to participant. d.
Uses appropriate communication skills for the frail older adult (hard of hearing, poor eye sight, aphasic, dementia, etc) e. Uses good listening skills. f.
Promotes independence in participant by offering choices and fostering self-help skills. g. Communicates respect and caring verbally and non-verbally with participant and other staff. h. Works cooperatively with other members of the interdisciplinary team to provide quality of care to LIFE Geisinger participants. i.
Follows assignment schedule and Participant plan of care. j.

COMPETENCIES AND SKILLS: Demonstrated personal care and homemaking.
Demonstrated sincere interest in working with frail elderly participants is a must. Demonstrated good communication skills. Demonstrated ability to write and speak English. Demonstrated ability to follow directions without constant supervision. Demonstrated ability to work as part of the multidisciplinary team and make meaningful contributions to the development of the plan of care. Performs duties in a self -directed manner with minimal supervision or direction. Ensures that routine and priority tasks are completed within established departmental time frames. Basic computer skills preferred. EDUCATION AND/OR EXPERIENCE: Graduation from high school or equivalent. Certification of Home Health Aide and/or Nursing Assistant in the state of Pennsylvania is preferred. Experience in personal care of an adult and homemaker service is required. Care of older adults is preferred in direct and indirect personal care. Prefer Home Health or Skilled Nursing Facility experience. Current CPR and First Aide certification. Must have certifications 6 months after date of hire. Valid Pennsylvania drivers license, reliable transportation with Auto insurance. Flexibility in the care of LIFE Geisinger participants needs.
