Chart Correction Specialist I
Accurate management of patient clinical data integrity in the Partners Electronic Health Record (EHR) is foundational to the care of all of the patients within the Partners HealthCare System (PHS) network. The Partners Healthcare patient database contains over 8 million patient records and serves as the foundation of care, patient communication and clinical coordination within the Partners network and beyond.
Under general supervision, this position is responsible for maintaining the integrity of the content of the Partners eCare/Epic electronic health record by performing in-depth investigations of questionable data entry and correcting as appropriate. The Chart Correction Specialist provides issue identification, assessment, resolution, and technical support in order to achieve desired outcomes and compliance with Partners Healthcare policies/procedures and standards.
This role analyzes chart correction scenarios and performs subsequent hands-on technical chart corrections as part of daily work responsibilities. This position serves as an organizational resource for issues involving chart correction, identity, and eCare instructional support.
The Chart Correction Specialist works at the center of the chart correction process, facilitating correction of clinical information throughout all inpatient and outpatient information systems. The individual coordinates the parties and interdisciplinary teams involved in each case (i.e., finance, clinical departments, and Pharmacy) and is responsible for ensuring all necessary corrections are made in a timely manner, and each case is validated prior to closure to ensure that documentation is complete and accurate to support patient safety and continuity of care.
Principal Duties and Responsibilities:
Corrects and resolves incorrect documentation issues within Partners entity and enterprise clinical systems such as fraudulent chart use, multiple patients' information in one health record, and one patient with two or more records/encounters in order to maintain a complete and correct legal health record for each patient.
Makes independent decisions on how best to address critical registration and documentation errors impacting patient safety and direct clinicians, ancillary & clinical departments on the necessary actions that must be taken to mitigate.
Facilitates emergent wrong registrations through the eCare Contact Move process on inpatients and emergency department patients, interfacing with and directing multiple stakeholders as to the appropriate actions to take to quickly resolve data quality issues as a result of wrong patients being registered in a manner and timeframe to minimize the impact to patient safety.
Ensures accurate correction of clinical information between patients and systems, confirming corrections with the senior members of the team. Precision and accuracy are mandatory due to high risk to patient safety.
Maintains In depth understanding of data flow between the EHR, various repositories and ancillary systems. Attain a general understanding of site level information systems both active and historical (legacy).
Develops and maintains good communication with a wide variety of internal and external professional relationships to accomplish departmental and hospital objectives. Communicate with customers throughout the chart correction process, utilizing e-mail, phone calls, in-basket messaging and Service Now updates.
Participate in analysis and correction of large scale system errors as a result of conversion, build issues and clinical system bugs.
Works with PHS/IS/eCare and Partners Site staff ensure smooth and coordinated correction to patient clinical data to ensure accuracy and prevent disruption to patient care and minimize the impact to patient safety.
Acts as a liaison between patients, clinicians, and practice/clinical staff ensure that incorrect data is identified, charting errors are communicated to the parties identified in the chart correction policy, erroneous data is corrected, and is verified that the clinical information was updated completely and correctly. Conduct patient interviews to verify clinical and demographic information to assist in record integrity corrections.
Communicates with clinicians, health care providers, and Partners IS Teams to provide critical and complex information on how to report and correct health record documentation, often in an urgent timeframe.
Processes patient requests for amendments to the clinical information in a patient's medical record, in accordance with federal (HIPAA) guidelines. Coordinates with Partners entities on multi-site corrections to facilitate a seamless (one patient, one record) patient experience. Makes addendum in the electronic record by following proper procedures and notifying all departments affected.
Documents outcomes of the chart correction process according to organizational policies. Advise clinicians, HIM staff, and other customers on HIM procedures and assist in resolution of incorrect documents and documentation related issues.
Provides feedback to PeC HIM team for changes and enhancements related to eCare HIM/Identity functionality. Assists the Pec Identity team members in preparing detailed specifications and proposals for modifying the existing system when the solutions follow clear precedence. Works with the EMPI and PeC HIM teams to review, test and implement new functionality for the EHR.
Assists in the development and implementation of policies and procedures to streamline the workflow of Chart Correction, increase productivity and maintain consistency with current practice.
Assists in training new Chart Correction employees and team members regarding Hospital and Departmental policies and procedures.
Coordinates with EMPI Operations Team or Site Data Integrity specialists when a duplicate or deactivate is identified or created during a chart correction.
Monitors and replies to all communication sent to Chart Correction team within the email box, in-basket and Service Now queue.

Chart Specialist-Lgmc Health Information Management-Full Time
Reconcile paper records with departmental documents to verify receipt of all records.
Confirm patient name, medical record number, and account number on every page in the record, front and back.
Identify and tape any torn edges.
Mount and tape down any sheets less than 8.5 by 11 inches on an 8.5-by-11-inch sheet.
For sheets with rhythm strips or other mounted documents, tape down the top of the strip so it does not catch in the automatic document feeder (if you will be scanning in "Portrait" mode).
Remove all staples.
Put tape over sticky materials.
For any documents on card stock or manila, such as Kardex or ambulance run sheets, make a photocopy before scanning.
Arrange multipage documents in date order, either chronological or reverse chronological, per department guidelines.
Ensure all pages are in the same proper orientation.
If a document is identified as likely to result in poor image quality once scanned, photocopy the document, adjusting the copy contrast (lightness/darkness). This may help improve the quality of the scanned image. If improvement is not noted, follow departmental guidelines for processing poor-quality originals.
Once all documents are prepped for scanning, bundle them together. Set them aside until you are ready to deliver them to the scanning staff.

Calibrate the scanner for proper image quality
"Fan" the stack of documents to be scanned in order to make sure the documents separate easily and that any previous hole punches or stapled pages do not stick together. Purpose: Reduce the number of automatic document feeder misfeeds.
Once records are scanned, remove them from the scanner's out tray. Begin process of verifying the quality and accuracy of the scans. With the records faceup, view the hard copy and the scanned image to confirm all images can be read and all pages were captured. This can be done by flipping through the hard copies as you verify each image, page by page. Purpose: Reduce the number of rescans as a result of poor image quality or skipped pages.
As images are reviewed, change the rotation, switch sides, etc., as needed. For example, documents that are typically viewed "landscape" should be rotated so they present to the end user in landscape mode. If a docu-ment was sent through the scanner so that the back page was scanned as the front page, switch sides to put the images in the appropriate order. Purpose: Improve the readability and use of the images by the end user.
If an image is identified to be of poor quality, adjust sensitivity or use the copy machine to enhance the quality of the document to be scanned. Purpose: Reduce the number of records returned for rescan as a result of poor image quality.
Quality Control/Validating Core Competencies

Demonstrate a commitment to ensuring responsible gaming and responsible alcohol service.
Works in assigned lounge area and provides fast, friendly, professional beverage and food services to all guests in accordance with departmental standards.
Presents the drink order to the bartender and writes check properly while waiting for drinks to be made.
Presents the drinks to the guest.
Obtains food orders and presents to kitchen personnel for preparation.
Obtains food when prepared and presents to guest.
Removes dirty dishes, glasses and silverware from table as needed.
Presents checks to the guest, accepts payment and gives change when necessary.
Ability to present change correctly and has knowledge of guest charge procedures.
Must be familiar with tray service, food menu and cocktail ingredients.
Performs any other duties as assigned.
Previous experience serving cocktails required. Ability to carry tray and stand for long periods. Good customer services and communication skills required. Must be able to obtain a NJ Casino Employee Registration License through the NJ Division of Gaming Enforcement.

Audits patient and insurance request audits
Educates and updates MASS software users of changes andrequirements; Responsible for processing MASS audit discrepancies and actionplans
Coordinates the correcting of discrepancies found in auditswith departments. Provides training to departments on charge issues foundduring audits
Reviews CCA daily edits and follow up with rebilling
Registered Nurse by the NCBON required, BSN preferred.
